Outpatient services for adolescents (age 12 to 18) and adults is designed to decrease barriers to treatment and allows you access to a team of clinicians who will complete a comprehensive assessment and talk with you about our services and available programs. Individuals can self-refer to PHP and IOP or get a referral from a provider or Doorway staff.It takes courage to face behavioral heath issues. Not only do participants receive intensive individual and group treatment, but strong aftercare and family components are also provided to ensure the continuity of care and the inclusion of family and support members in the treatment process. Personalized plans are developed to address their substance use, mental health, and primary care needs, as well as the major social determinants of health, poverty, food scarcity, and access to reliable housing and transportation, among others. The teams coordinate closely with primary care providers on a biweekly basis to ensure that all aspects of patients’ wellbeing and health are addressed appropriately.Īt the beginning of the program, all participants are assessed with a variety of standardized tools, such as AUDIT/DAST, PHQ 2 or 9, the Addiction Severity Index, the Columbia Suicide Risk Assessment and the URICA (University of Rhode Island Change Assessment).
Depending on their role, staff can provide case coordination, assist participants in accessing community-based services, provide medication management, evaluate participants for medical issues, and more. PHP and IOP are for individuals who need care that is more intensive than individual outpatient programs but not as intensive as residential treatment.
